Job Description:
  • Work within the Home Health department to help with care transitions where you will perform a variety of patient centered administrative tasks
  • Support/consult patients and families (10 to 20 cases per day) with their specific discharge plan and plan of care for home health referrals 
  • Act as a Liaison between outside agencies, outpatient setting and the inpatient setting.
  • Provides high degree of customer service to patients/families, case management teams, physician/providers, and clinical team
  • Provide administrative support such as Faxes, Copies, phone calls as directed in a timely manner
  • Enter appropriate data into Epic, such as physician/provider orders, home health and hospice orders/information and documentation, and other clinical and non-clinical information
  • Retrieve voice mails and convey messages to staff in a timely and accurate manner; triage priority requests appropriately

Job Information:
  • Location: Everett, WA (100% On-Site)
  • Assignment Length: 6+ months
  • Working Hours: 8: 00AM-5: 00PM (Monday-Friday) or 8: 00AM-7: 00PM (Monday, Tuesday, Thursday, Friday)
  • Pay Rate: $25.00-$28.00




Candidate Requirements:
  • Previous work experience in a patient-facing Healthcare environment
  • Strong computer skills; use of Microsoft Office suite and electronic medical record (EPIC)
  • Proven ability to convey and document pertinent information clearly
  • Demonstrated ability to develop strong collaborative working relationships
  • Excellent verbal and written communications skills in English
  • Understands how to provide high levels of customer service by phone, email, and face-to-face
  • Communicates clearly, sensitively and in supportive professional manner with patient/family/caregivers, co-workers and public
  • Demonstrated ability to maintain positive, collaborative, and constructive interpersonal relationships. Understands and practices the principles of effective teamwork.
